To assist with event planning, please use this form to provide the names of your attendees, note any dietary restrictions or special meal requests, and indicate whether any of your sponsorship seats will be donated for first responders to attend at no charge. 

Please complete this form no later than August 17. This deadline will allow us to provide final attendance numbers and dietary accommodations to the Hilton staff by their required deadline.
